The Concussion Care Network includes different regulated health care professionals with over 40 years of combined concussion experience. Interdisciplinary care is best-practice for concussion treatment, because concussion impacts so many aspects of our function after injury. Referrals to the Concussion Care Network can be made by your family physician, nurse practitioner, other allied health professionals (e.g. chiropractor, physiotherapist) or you can refer yourself. If you are being referred by a health care practitioner, they can use this referral form by clicking here. To refer yourself or a family member, contact us directly by phone or email
